Field RN Supervisor

The Field RN Supervisor is responsible for the nursing supervision of personal care services in accordance with the policies and procedures of the New York State Department of Social Services Regulations 505.14.

QUALIFICATIONS

• Graduate of an accredited school or college of Registered Professional Nursing
• Current New York State Registered Professional Nurse licensure
• At least three years experience as a registered nurse, preferably in the provision of home care services
• Two recent references indicating satisfactory performance
• Proof of valid Mal-practice Insurance
• A completed health examination signed by a physician and in compliance with the agency policy on employee health requirements

JOB KNOWLEDGE

• Knowledge of what is involved in the delivery of home care services to the elderly, chronically ill or disabled patient
• The ability to identify symptoms and signs of current or potential health problems of the patient
• Ability to assess the patient for the capacity to manage safely in the home environment and the potential for safety hazards
• Have the aptitude to supervise, direct and evaluate the performance of the Personal Care Aide

RESPONSIBILITIES

• Supervisory visits made according to the agency policy and Nassau County Department of Social Services PEA Unit
• Development and revision of the Plan of Care according to the policy and procedures of the Nassau County Department of Social Services, PCA Unit
• The condition of the patient is assessed during every visit and any changes or problems are reported to the Patient Care Director
• Evaluates the Personal Care Aide’s competency and adherence to the Care Plan.
• Provides on-the-job training as needed in a specific skill, procedure or technique
• Reports patient problems and changes in condition to the agency administration
• Written reports are submitted in a timely manner according to agency policy
• Collaborates with other members of the administration team and members of the Quality Improvement Committee
• Participates to compliance with requirements of all federal, state, and local regulations
• Participates in the agency Performance/Quality Improvement Programs
• Performs all other duties as assigned
